WHAT IS FORM?
FORM is a discipleship, leadership, and missional training space for people who are in the midst of life transition, especially with young adults ages 18-29 and retirees in mind. It is a life-changing 7-months of self-discovery, hearing from God, and being trained for a lifetime of following Jesus in an increasingly post-Christian culture.
You will be trained in the core tools, language, and philosophy of missional discipleship in an increasingly post-Christian culture and get hands-on practical experience alongside Apex leaders and staff in the work of our house church network. You will be immersed in scripture and the lifestyle of Jesus in order to prepare you for a life of living out your faith in all of life (family, church, workplace, and community). We'll help; you gain the tools to live as Jesus has called us to life: seeking God, loving one another, and making disciples.
WHAT IS THE Time Frame and STRUCTURE OF FORM?
Applications will be received from now through September 22, 2019.
Begins October 6, 2019 including the Learning Community Immersion October 12-13 and Spring 2019 Immersion. Ends May 7, 2020.
Gather together every Sundays (8am-10am, with gathering involvement expected) and Thursdays (8:30am-12pm) at Apex Kettering for prayer, teaching, huddle, training, and practical ministry experience.
Six Saturday Outreach experiences serving the Greater Dayton area.
Two Weekend Immersions, learning along with House Church leaders.
Regular engagement with the Apex Kettering staff in particular areas of ministry.
*If you are a student who already has class on Thursday mornings, don’t let that deter you. We can make the content available to you for the Fall, but you can join us in the Winter if you keep Thursday mornings open when you schedule classes.
What is THE teaching and training Content?
Form participants will be trained in the core DNA of tools, language, and philosophy of missional discipleship that church leaders and missionary organizations are receiving all over the world. It is deeply rooted in Scripture, the lifestyle of Jesus, and contextually tailored to living in an increasingly post-Christian culture.
WHO WILL BE INVESTING IN THE FORM PARTICIPANTS?
Members of the Growing team at Apex will oversee the program and help train and disciple the participants. Other leaders will invest both formally and informally.
IS IT POSSIBLE TO WORK OR GO TO COLLEGE DURING THE FORM YEAR?
Yes. The structured aspect of Form takes place during set hours on Sunday/Thursday, therefore, participants can work around that to take classes and/or work.

IS FORM SPECIFICALLY FOR THOSE PREPARING FOR FULL-TIME VOCATIONAL MINISTRY?
No. Form is intended to give an intensive training environment for a year to prepare you for a life-calling of missional discipleship in their community, church, and any vocation.
ARE THERE OTHER FORM EXPRESSIONS ELSEWHERE?
WHAT ARE THE REQUIREMENTS FOR BEING ACCEPTED?
One must be a growing believer in Jesus, agree to a general statement of faith, fill out an application, submit two character references, have access to personal transportation, have a high school diploma, and pass the interview process. Applications will be received from now through September 22, 2019. We recommend submitting your application as soon as possible so that applicants can be reviewed and accepted in a timely fashion.
